1. By using your Discount Card issued under the Healthcare Assist (“Healthcare Assist”) discount program, each applicant or member (each called a "Member") accepts and agrees to be bound by the Healthcare Assist discount program Terms and Conditions applicable from time to time.
   
2. The Healthcare Assist Discount Card ("Discount Card") is valid at any participating Preferred Partner ("Preferred Partner").
   
3. The Discount Card is not a credit or charge card, is not transferable and remains the property of Healthcare Assist. If a Member wishes to obtain any additional Discount Cards for family members those additional Discount Cards will be issued at a cost of $4.95 ($4.50 + GST) per card.
   
4. If new Healthcare Assist Members have not received their Discount Card within two weeks they are to call 1300 725 909 to complete a replacement card request.
   
5. Members will receive discounts on their total bill from Preferred Partners other than products and services for which the Member is already entitled to receive a discount other than by reason of the Healthcare Assist discount program.
   
6. Preferred Partners will offer discounts on all products, services and treatments, other than the exclusions shown in section 7, at the following rates:

   Optical Assist	20%
   Dental Assist	15%
   Vet Assist	15%
   Medical Assist	10%
   Pharm Assist	10%
   Natural Assist	10%

   
   
7. Preferred Partners may exclude products and services from being eligible for a Healthcare Assist discount with prior consent from Healthcare Assist. Current exclusions include pet foods, consultations which have been bulk billed through Medicare, and PBS medicines where the Australian Government do not allow discounts.
   
8. Subject to anything else contained here all Memberships will have a life of 12 months. Healthcare Assist may cancel or bring forward the expiry date of any Discount Cards at any time in its absolute discretion without reason or notice.
   
9. Discount Cards are not transferable and can only be used for discount redemption by the person to whom the Discount Card has been issued.
   
10. If a Preferred Partner elects to resign from the Healthcare Assist program, all products and services sold after four months from the date of the resignation will not be eligible for discount redemption even if in the middle of treatment.
    
11. To the extent permitted by law, Healthcare Assist or any participating Preferred Partner will not be liable for any errors, omissions, delays or disruptions in the operation of the Healthcare Assist discount program.
    
12. Healthcare Assist will not be liable in any way if any participating Preferred Partner fails to provide a Healthcare Assist discount on any products, services and or treatments. Healthcare Assist does not accept any liability in connection with the failure or refusal of any participating Preferred Partner to provide products, services or treatment. Healthcare Assist does not accept liability or responsibility for any workmanship, the quality or suitability of products and services offered by our Preferred Partners.
    
13. Members must promptly notify Healthcare Assist of any change of their address or if their Discount Card is lost or stolen. Healthcare Assist or any Preferred Partner is not liable for the delay in providing or replacing the Discount Card or for any unauthorised use of the Discount Card. Replacement cards will be issued at a cost of $4.50 + GST) per card, unless a copy of a police report verifying the card was stolen is forwarded with the application.
    
14. Healthcare Assist may replace or amend these Terms and Conditions at any time at its absolute discretion without having to provide any reason. Healthcare Assist will use its best endeavours to notify Members of any replacement or amendment but will not be held liable if it does not.
    
15. Healthcare Assist may suspend or terminate the Healthcare Assist program or a Member's membership in the Healthcare Assist program at any time without reason or notice and on terms and conditions as Healthcare Assist in its sole discretion determines. Healthcare Assist will not be liable for any loss or damage that may arise from any suspension or termination.
    
16. No Preferred Partner has any authority, express or implied, to may any representation, warranty or statement on behalf of Healthcare Assist or in relation to  the Healthcare Assist program and Healthcare Assist accepts no liability in respect of any representation, warranty or statement made by any Preferred Partner.
    
17. Members acknowledge that a participating Preferred Partners and not Healthcare Assist will supply products, services and treatment. Accordingly Healthcare Assist does not give any guarantee, undertaking or warranty concerning the products, services or treatments supplied or the performance of any participating Preferred Partner. All conditions and warranties, whether express or implied and whether arising under statute or otherwise, as to the condition, suitability, quality, fitness or safety of any goods, services or treatments supplied by any participating Preferred Partner are expressly excluded to the full extent permitted by law. As such please refer to the terms and conditions displayed by our Preferred Partners.   Members acknowledge that if they have any complaints with Preferred Partners that they notify Healthcare Assist so that can be investigated by Healthcare Assist.
    
18. Information concerning Members, including information contained in the joining application form will be held in a database for Healthcare Assist administering the Healthcare Assist program, a telephone service centre, producers of Cards and data processors for the Healthcare Assist program. Members consent to information from that database also being made available to and used by Healthcare Assist for marketing, planning, product development, research and other purposes relating to the Healthcare Assist discount program. The Healthcare Assist discount program is void where prohibited or restricted by law.
